Community Health Care Specialist

As a Community Health Care Specialist with our innovative Western Canadian organization, you will be part of a passionate team that empowers customers and communities to adopt healthier lifestyles through quality, patient-centred care. Our pharmacy teams strive to deliver exceptional services in long term care facilities, ensuring compliance with regulations, policies, and procedures.
We are currently seeking a highly skilled pharmacist to join our Continuing Care Division location in Victoria, BC. Key responsibilities include:
- Providing pharmaceutical services to patients in continuing care facilities
- Ensuring compliance with regulatory requirements, company policies, and procedures
- Supervising pharmacy assistants and regulated pharmacy technicians, as required
- Participating in the on-call rotation for after-hours consultation to our contracted homes
- Providing medical information/counselling to patients, care providers, and other healthcare professionals
To excel in this role, you will require:
- A Canadian Pharmacist licence in good standing with the respective College of Pharmacists
- Minimum one year of pharmacist experience, preferably in long term care or specialty pharmacy
- Drug Administration Certification with the College of Pharmacists of BC
- Completion of the CPBC Methadone Maintenance Treatment (MMT) Program and OAT-CAMPP workshop
- Proficiency with Pharmacy Software solutions, preferably Kroll
