Community Health Services Coordinator

About the RoleThe Registered Nurse (RN) Community Health Services practices in accordance with relevant provincial legislation, nursing standards, and codes of ethics as outlined by the BC College of Nurses and Midwives and the Canadian Nurses Association (CNA). Employer policies and individual competencies also guide practice.
This RN provides nursing care according to established organizational policies, procedures, guidelines, and controls on practice. The role involves working in various clinical settings and collaborating with clients, healthcare teams, and primary care providers.
A community-focused approach ensures client-centered care for achieving and maintaining maximum health through prevention, treatment, and palliation of illness and injury. Initial and ongoing assessments, planning, implementation of interventions, and coordination of health services are essential components of this role.
Key qualifications include:
- A valid BC driver's license
- Ability to provide client-focused, trauma-informed, and culturally safe care
- Capacity to complete initial and ongoing client assessments and provide nursing care through evidence-based guidelines
- Ability to instruct clients and others about topics essential to healthcare and well-being
- Effective time and resource management
- Knowledge of care planning and care coordination, including community programs and services
- Demonstrated knowledge of community health practice, chronic disease management, geriatric, and palliative nursing practice within a client and caregiver-centered care approach
