Job Summary
This full-time position involves providing nursing care to patients in their homes, working closely with our interdisciplinary team to deliver comprehensive care that meets each patient's unique needs.
The ideal candidate will possess excellent communication and interpersonal skills, be able to work independently, and have a strong knowledge of community services and resources.
Key Responsibilities:
- Provide comprehensive nursing care to patients, including history taking, assessments, medication management, and wound care.
- Develop and implement individualized care plans in collaboration with the interprofessional team.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and industry developments.
What We Offer
A competitive salary commensurate with industry standards ($85,000 - $110,000 per year)
