About the Role

We are seeking a skilled Registered Practical Nurse to join our team in a community nursing role. As a RPN, you will provide high-quality care to patients in various settings, working collaboratively with RNs to support complex patients.

Key Responsibilities
  • Participate in scheduled visits to assess patient needs and provide care within your scope of practice
  • Deliver safe and ethical nursing care, using appropriate assessment tools and techniques
  • Develop and implement care plans, monitoring patient progress and adjusting as needed
  • Build rapport and trust with patients, engaging them in their care and involving them in decision-making
  • Maintain accurate and up-to-date patient records
Requirements
  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in community care is an asset
  • Reliability, flexibility, and a willingness to travel to patient residences
  • Passion for making a difference in your community
