Nurse Practitioner

Fraser Health is seeking a highly skilled Registered Nurse to join our team in the designated clinical area. As a key member of the inter-professional care team, you will provide direct patient care by assessing, planning, providing, and evaluating nursing care. Your expertise will be instrumental in teaching patients and families about illness management, prevention, and treatment.
Key Responsibilities- Conduct comprehensive nursing assessments of patients and unborn babies on admission to the outpatient clinic or home environment setting.
- Collaborate with the inter-professional care team to identify and resolve issues, changes in patient condition, and adjustments to care.
- Develop and implement care plans in collaboration with patients and their families/caregivers.
- Provide individual counseling, group facilitation, lectures, and follow-up on disease-modifying therapies.
- Assess learning needs of patients and families/caregivers and provide information and demonstration of care activities.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years of recent related clinical experience in the designated clinical area, and completion of postgraduate coursework and/or certification applicable to the designated clinical area or an equivalent combination of education, training, and experience.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes as required.
- Clinical proficiency in the designated clinical area.
- Ability to complete initial and ongoing patient assessment and apply nursing theory and expertise to provide nursing care through appropriate/prescribed technical, therapeutic, and safety interventions.
- Ability to recognize acuity changes and use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
- Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the inter-professional care team using verbal, written, and electronic communication means.
