Crisis Intervention Specialist

WoodGreen Community Services is seeking a skilled Crisis Specialist to join our team. As a Crisis Specialist, you will work with seniors who are experiencing mental health and substance use issues, providing short-term intensive case management services and ongoing support.
Key Responsibilities:- Participate in crisis shifts and visit clients in their homes or community locations with a partner.
- Perform psychosocial and geriatric assessments to identify client needs and develop a care plan.
- Provide case management services, including service navigation, care coordination, and follow-up support.
- Develop and implement a care plan to address client priority and emerging needs.
- Provide supportive counseling and advocacy support to clients and their families.
- Work collaboratively with hospitals, community professionals, and other referral sources to ensure a continuum of care.
- Maintain accurate records and documentation, and collect statistical data as required.
- Masters of Social Work or post-graduate degree in a related field, with 2+ years of experience in crisis support and working with complex seniors.
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers or another relevant regulatory body.
- Related experience in crisis support and demonstrated experience working with complex seniors.
