Job Title: Crisis Specialist

WoodGreen is seeking a skilled Crisis Specialist to join our team in providing crisis support and intervention services to seniors with complex needs.

Job Summary

The Crisis Specialist will work in an inter-agency, multi-disciplinary team to provide crisis support and intervention services to seniors with physical, cognitive, mental health, and other concerns. The successful candidate will have a strong background in crisis support and intervention, with experience working with complex seniors.

Key Responsibilities
  • Participate in crisis shifts and visit clients in their homes or community locations with a partner.
  • Perform psychosocial and geriatric assessments to identify client needs and develop a care plan.
  • Provide short-term intensive case management services to a caseload of seniors with mental health and substance use issues.
  • Assess risk in crisis situations and develop an intervention and action plan.
  • Provide service/care coordination, system navigation, and follow-up support to clients.
  • Develop and implement a care plan to address client priority and emerging needs.
  • Provide supportive counseling to seniors and older adults using a wide range of therapeutic approaches.
  • Participate in case conferences and clinical meetings to ensure coordination of services and a continuum of care.
  • Provide advocacy support and advocate with clients and/or their family or caregivers.
  • Work closely with hospitals and other referral sources to support clients and facilitate smooth transitions.
Requirements
  • Masters of Social Work or post-graduate degree in a relevant field with 2+ years of experience in crisis support and intervention.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers or another relevant regulatory body in Ontario.
  • Experience working with complex seniors and a strong background in crisis support and intervention.
