As a community nurse with CarePartners, you will have the opportunity to exercise your full scope of practice, pursuing specialization and championing innovative health care programs. Your responsibilities will include:
- Developing and maintaining effective care plans with respect to community health services
- Providing exceptional nursing services to patients in their homes and communities
- Communicating and liaising with patients, family members, and other healthcare professionals
To be successful in this role, you will require:
- A valid driver's license and vehicle
- Excellent English verbal and written communication skills
- A current immunization record, including a 2-step TB test
