$62,000 - $90,000 per year, depending on experience and qualifications.

Job Description:
We are seeking a skilled Home Health Nurse to join our team as a Community Care Specialist. This role will involve providing high-quality nursing services to patients in their homes, as well as collaborating with interdisciplinary teams to develop effective care plans.

Key Responsibilities:
• Provide exceptional nursing care to patients in their homes, including assessing their needs and developing personalized care plans.
• Collaborate with patients, family members, and healthcare professionals to ensure seamless care coordination.
• Conduct thorough assessments of patients' medical and social needs, identifying areas for improvement and developing strategies to address them.
• Communicate effectively with patients, families, and healthcare teams to provide updates and resolve issues promptly.
• Participate in continuous learning and professional development to stay up-to-date on best practices and evidence-based care.

Requirements:
• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
• Current College of Nurses of Ontario (CNO) registration in good standing.
• Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
• Valid driver's license and reliable vehicle, as regular travel is required.
• Excellent English verbal and written communication skills.
• CPR Certification and current immunization record, including 2-step TB test.
• Vulnerable sector check and clear background check.
