Job Summary

CarePartners is seeking a registered nurse or registered practical nurse to join our team as a community health nurse in the Sundridge area. This full-time position offers flexible scheduling and work-life balance.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide high-quality nursing services to patients in their homes, including virtual visits and one-on-one care
  • Communicate effectively with patients, family members, and healthcare teams to ensure seamless care coordination
Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A valid driver's license and reliable vehicle for regular travel
