Primary Care Network Lead

The EightSix Network is seeking an experienced Primary Care Network Lead to join our team on the North Shore. This is a key leadership role that will play a critical part in shaping the future of primary care in our community.
About the Role:
This position is responsible for developing and implementing strategies to improve patient outcomes, enhance collaboration among healthcare providers, and increase access to primary care services. The ideal candidate will have a strong background in project management, excellent communication and interpersonal skills, and the ability to work effectively with diverse stakeholders.
Responsibilities:
- Develop and implement plans to improve patient care and outcomes
- Collaborate with healthcare providers, community organizations, and government agencies to increase access to primary care services
- Manage and coordinate projects related to primary care network development and implementation
- Provide leadership and guidance to staff and stakeholders to achieve program goals and objectives
- Monitor and evaluate program progress and make recommendations for improvement
- Build relationships with key stakeholders, including physicians, nurses, and other healthcare professionals
Requirements:
- Masters degree in a relevant field (e.g. public health, health administration)
- Minimum 5 years of experience in a leadership role in primary care or a related field
- Excellent communication and interpersonal skills
- Ability to work effectively with diverse stakeholders
- Strong analytical and problem-solving skills
- Experience with project management principles and methodologies
Salary Range: $90,000-$120,000 per year
