Job Description
The Primary Care Nurse Specialist plays a vital role in our clinical team, providing comprehensive care for clients/patients with complex health needs. This includes conducting assessments, developing and implementing care plans, and collaborating with the interdisciplinary team to achieve optimal outcomes.
Responsibilities
- Conduct comprehensive assessments to identify client/patient needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to implement care plans and ensure continuity of care.
- Provide education and support to clients/patients to promote healthy behaviors and self-care.
- Liaise with healthcare professionals to coordinate care and address client/patient needs.
- Participate in quality improvement initiatives to enhance patient-centered care.
Requirements
- Current registration with the BC College of Nurses and Midwives (BCCNM) as a practicing RN registrant or RPN registrant.
- Two years' recent related experience working with diverse marginalized populations in healthcare settings or community-based programs.
- Demonstrated knowledge of trauma-informed care, violence prevention, transgender care, harm reduction, recovery-oriented practice, and indigenous cultural competency training.
- Comprehensive triaging skills for physical health and psychosocial needs.
- Ability to communicate effectively both verbally and in writing.
- Physical ability to carry out the duties of the position.
Compensation
We offer a competitive salary of $95,000 - $110,000 per year, depending on experience.
