Job Summary

We are seeking a highly skilled and compassionate Field Case Manager to join our team at CB Canada. As a Field Case Manager, you will play a critical role in providing exceptional patient care and support to our clients.

Key Responsibilities
  • Patient Support
    • Develop and implement personalized patient care plans to ensure optimal health outcomes
    • Provide education and guidance to patients and their families on disease management and treatment options
    • Coordinate with healthcare providers to ensure seamless care transitions and timely access to therapy
    • Identify and address patient barriers to care, including financial and social determinants
  • Practice Management
    • Collaborate with healthcare providers to customize program elements and ensure adherence to clinical standards
    • Review patient charts and assess previous therapies and tests to complete Special Authorization petitions
    • Proactively assess programs and make suggestions to enhance outcomes and quality of service
  • Mentorship and Peer Support
    • Assist in training and development of new Field Case Managers as needed
    • Provide regular and ongoing communication and support to peers
    • Develop and maintain positive relationships with the Patient Support team
  • Client Interface
    • Participate in scheduled KOL physician meetings with Sales Representatives
    • Develop strong working relationships with physicians and office staff to ensure smooth maintenance of patients enrolled in the program
    • Proactively provide assistance to resolve client issues related to assigned physicians
  • Other
    • Complete all relevant reports and documentation as per specified timelines and standards
    • Perform other duties and tasks as required by the organization
Requirements
  • Education and Experience
    • RN or RPN registration with the Provincial College of Nurses
    • Minimum three (3) years nursing experience in a clinical setting
    • Available to travel within an assigned territory, with some overnight travel possible
  • Skills and Knowledge
    • Strong customer service and communication skills
    • Ability to work independently and as part of a team
    • Excellent analytical and problem-solving skills
    • Intermediate knowledge of Microsoft Office programs
