Position Overview
As a Field Nurse Case Manager, you will be instrumental in assisting patients and healthcare professionals through our Patient Support Program (PSP). This distinctive nursing role entails delivering tailored administrative and clinical support to both physicians and patients, ensuring a smooth treatment experience.
Key Responsibilities
This field-based role involves travel and offers avenues for professional development and advancement. Your duties will encompass:
- Acting as the primary liaison for prescribers to enroll patients in the program
- Establishing and nurturing robust relationships with key stakeholders
- Conducting in-office patient chart assessments for reimbursement assistance
- Providing patient education on disease management and medication, including injection training and adherence support
- Collaborating with internal teams to facilitate the patient journey
- Ensuring high levels of customer satisfaction and addressing unmet needs
- Participating in ongoing medical education events
Qualifications
To be considered for this position, you must possess a nursing degree, be registered with the College of Nurses of British Columbia, and have experience in hospital environments or Patient Support Programs.
