Empathetic Support Specialist

Key Responsibilities:
- Develop and maintain positive relationships with service users, advocating for their rights and choices as needed.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and exceptional support.
- Cultivate empowering environments that encourage service users to thrive in their communities.
- Participate in creating, implementing, and documenting Personal Service Plans tailored to each individual's needs.
- Maintain excellent communication with family members, professionals, co-workers, and community stakeholders.
- Provide assistance with daily living activities and medication administration while upholding the highest standards of care and safety.
- Foster a deep understanding of behavior management principles and strategies, including mental health considerations.
- Accurately record daily activities and personal health information (PHI) on our case management database.
- Work collaboratively with service users to ensure their living space remains clean and well-maintained, including meal preparation and light housekeeping duties.
Required Qualifications:
- Diploma or equivalent in Community Support Work or related field.
- Minimum 2 years of experience in community social services or healthcare.
- Current Emergency First Aid/CPR and Food Safe certifications.
- Previous training in behaviour management courses.
- Excellent verbal and written communication skills.
- Non-verbal communication skills.
- Restricted class 4 driver's license (for full-time positions).
- Satisfactory Driver's Abstract.
- Criminal record clearance for children and vulnerable adults.
- Proof of COVID-19 vaccination status.
- TB Test.
- Moderate computer literacy.
- Eligibility to work in Canada.
