Job Summary

We are seeking a skilled Home Health Registered Nurse Liaison to join our team at Peace Arch Hospital in White Rock, BC.

This role involves performing client assessments and acting as a liaison between the acute care facility and community care service to facilitate discharge planning. The successful candidate will assess clients' care needs, arrange for referrals to community services, and collaborate with other health care professionals to provide advice on discharge options.

The ideal candidate will have recent experience in acute care and/or community nursing, excellent communication skills, and the ability to problem-solve and prioritize client caseloads.

Key Responsibilities:

  • Arrange client referrals from staff within an assigned acute care facility that requires community care services upon discharge or a change in level of service.
  • Interview and screen clients, using various assessment tools to determine appropriate care plans.
  • Develop and implement client care plans with interdisciplinary teams, ensuring timely discharge and access to resources.
  • Liaise with clients, families, physicians, and other healthcare professionals to arrange community services.
  • Facilitate timely exchange and provision of information regarding client care between acute care facilities and community services.

Qualifications:

  • Graduation from an approved school of Nursing.
  • One year recent related clinical experience in acute care and/or community nursing.
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
