About the Role


Fraser Health is seeking an experienced Social Worker to join our Acute Care team. As a Social Care Specialist, you will provide comprehensive therapeutic clinical counselling services to patients and families in the assigned clinical specialty area.



Key Responsibilities

  • Conduct comprehensive social work and psycho-social assessments of patients and families, using various modalities and professional standards.
  • Develop and coordinate care plans, including discharge planning and hospital stay, in collaboration with interdisciplinary teams.
  • Educate and consult with patients, families, and healthcare professionals on high-risk factors, recovery, and community practice relevant to the assigned clinical specialty area.
  • Maintain accurate patient records, documenting assessments, charting information, and preparing progress notes and reports as required.
  • Communicate effectively with community health and social agencies, making referrals and exchanging relevant information.
  • Participate in program evaluation, identifying opportunities for change, assessing available resources, and recommending solutions.


Requirements

  • Masters Degree of Social Work from an accredited school of Social Work.
  • Current full registration with the BC College of Social Workers.
  • Ability to initiate, develop, and carry out research activities.
  • Demonstrated ability to communicate effectively with patients, families, staff, and community agency personnel.
  • Ability to organize and prioritize workload in a complex environment.

Estimated Salary

The estimated salary for this position is $85,000 - $110,000 per annum, depending on qualifications and experience.
