About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Continuing Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for assessing, planning, and coordinating care for clients referred to Continuing Care. Your expertise will help individuals and families navigate complex health situations and access the resources they need to thrive in their communities.

As a Continuing Care Coordinator, you will work closely with interprofessional teams to facilitate hospital discharge and ensure a smooth transition back to the community. Your strong assessment and communication skills will enable you to identify clients' goals, unmet needs, and resource requirements, and develop personalized care plans to meet their unique needs.

You will be responsible for authorizing services, making referrals, and linking clients with community resources to support their independence and well-being. Your ability to work independently and collaboratively in a fast-paced environment will be essential in this role.

About You
  • Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and experience in dedicated case management or discharge planning
  • Excellent assessment, interviewing, and negotiation skills
  • Competent with computer use, keyboarding, and ability to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Valid driver's license and access to a reliable vehicle
Hours and Compensation

Permanent Full-time/ 70 Hours Bi-weekly

Shifts may include days, evenings, weekends, and holidays

$46.75 Hourly

Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
