We are seeking a skilled Registered Practical Nurse (RPN) to join our team in providing high-quality community care to patients in various settings. As a RPN, you will work independently and collaboratively with RNs to deliver care to patients of all ages with differing complexity of care needs.
Key Responsibilities:
- Participate in scheduled visits in the community to assess patient needs and provide care within your scope of practice.
- Provide safe and ethical nursing care, using appropriate assessment tools and techniques to assess patients' individual needs.
- Execute and continuously monitor care plans, adjusting as required to ensure optimal patient outcomes.
- Engage patients using a collaborative approach, building rapport and trust to promote positive health outcomes.
- Maintain and update professional patient records related to patient encounters and plans.
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in delivering community care is an asset.
- Reliability and flexibility to meet patient care needs.
- Willingness to travel to and from patient residences within a designated area.
- Experience working with vulnerable populations.
- Passion for making a difference in your community.
