About the Role

Vancouver Coastal Health is seeking a skilled Outreach Worker to join our Overdose Outreach Team in Vancouver's Downtown Eastside.

Key Responsibilities
  • Provide support services to individuals and their families accessing opioid agonist therapy and related support services.
  • Monitor clients' health conditions and environments.
  • Arrange and escort clients on appointments, errands, and activities.
  • Create linkages to community resources and programs.
  • Assist clients in accessing benefits and provide brief planning and coordination on substance use, opioid agonist therapy, overdose prevention, and harm reduction.
  • Administer medication to clients and provide medication reminders under the direction of a Registered Nurse.
  • Liaise with community resources and healthcare providers.
  • Provide basic health-promoting information and promote life-skills development.
  • Perform general clerical duties, attend meetings, and participate in patient care planning and coordination.
About the Team

Our team of outreach and social workers and peers provides connections for people who have recently experienced opioid overdose and/or are at high risk for opioid overdose to substance use care and support.

Qualifications
  • Diploma in Community Social Services and one year of related experience working with people who use substances and those suffering from complex mental health issues.
  • Valid BC driver's license and vehicle.
  • Cardiopulmonary Resuscitation (CPR) certification.
