Outreach Worker

Vancouver Coastal Health is seeking an experienced Outreach Worker to join our Overdose Outreach Team in Vancouver's Downtown Eastside. As a key member of our interdisciplinary team, you will provide vital support services to individuals and their families, helping them access opioid agonist therapy and related support services.
Key Responsibilities- Work collaboratively with clients and their families to identify and address their unique needs and challenges.
- Provide education and support on harm reduction principles, overdose prevention, and substance use recovery.
- Assist clients in accessing benefits and assistance, and provide brief planning and coordination on issues related to substance use and opioid agonist therapy.
- Under the direction of the Registered Nurse, administer medication to clients and provide medication reminders.
- Liaise and work with community resources and healthcare providers to ensure seamless care coordination.
- Provide basic health-promoting information and promote development of life-skills to clients.
Our team of outreach and social workers, and peers, provide connections for people who have recently experienced opioid overdose and/or are at high risk for opioid overdose to substance use care and support. We help clients navigate to appropriate services, support in accessing Opioid Agonist Therapy (OAT), and overdose prevention education.
Qualifications- Diploma in Community Social Services and one (1) year related experience working with people who use substances and those suffering from complex mental health issues.
- Valid BC driver's license and vehicle.
- Cardiopulmonary Resuscitation (CPR).
