About the Role

We are seeking a skilled Registered Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to clients in their homes and communities.

Key Responsibilities
  • Conduct comprehensive client assessments to identify their unique needs and develop personalized care plans.
  • Implement and evaluate care based on best evidence and practices, ensuring the highest level of client satisfaction and outcomes.
  • Collaborate with other healthcare professionals and community support services to ensure seamless care transitions and optimal client outcomes.
  • Provide education and support to clients and their families to promote healthy living and independence.
Requirements
  • Current registration with the College of Nurses of Ontario.
  • Canadian Nurses Association (CNA) specialty certification an asset.
  • Enthusiasm and love of community care and passion for evidence-informed practice and excellence in care.
  • A current CPR certificate is required.
  • A valid driver's license and vehicle (travel required).
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
