About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Transitions team at SE Health. As a Nurse Care Coordinator, you will play a vital role in providing high-quality care to our clients in the community.

Key Responsibilities
  • Provide comprehensive care to clients in an interdisciplinary team setting
  • Act as the primary contact for clients and lead care planning discussions
  • Develop and implement individualized care plans based on client needs and best practices
  • Facilitate regular team communication and collaboration to ensure seamless care delivery
  • Support clients through care transitions by connecting them with community resources
  • Develop strong relationships with clients, families, and caregivers to promote self-care and health optimization
Requirements
  • Registered in good standing with the College of Nurses of Ontario
  • Bachelor's Degree in nursing preferred
  • Canadian Nurses Association (CNA) community nursing certification is an asset
  • Minimum of two years of nursing experience
  • Experience in home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
  • Demonstrated clinical leadership experience and excellent assessment skills
  • Ability to work independently and in a team environment
  • Excellent communication and interpersonal skills
