Nurse Care Coordinator

We are seeking a skilled Registered Practical Nurse to join our team at Southbridge Care Homes. As a key member of our healthcare team, you will be responsible for providing high-quality care to our residents and supporting the overall well-being of our community.
Key Responsibilities- Assess the needs of individual residents and develop personalized care plans to promote maximum independence.
- Supervise and guide attendants and unregulated care providers to ensure high-quality care is delivered.
- Administer medication and provide direct resident care in accordance with current standards and policies.
- Maintain accurate records of nursing observations and procedures performed.
- Collaborate with medical and allied staff to ensure seamless care delivery.
- Current Ontario Certificate of Competence from the College of Nurses of Ontario.
- Medication administration certificate required.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
