**Job Summary**

Southbridge Care Homes is seeking a skilled and compassionate Registered Practical Nurse to join our care team. As a key member of our nursing staff, you will play a vital role in providing exceptional resident care and promoting a positive living environment.

**Key Responsibilities**

  • Deliver high-quality nursing care to residents, adhering to established protocols and standards of care.
  • Conduct regular assessments and evaluations to identify resident needs and develop individualized care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication.
  • Provide education and training to residents, families, and staff on various aspects of care and wellness.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

**Requirements**

  • Current registration with the College of Nurses of Ontario.
  • Previous experience in long-term care or a related field, with a strong understanding of gerontological nursing principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Ability to prioritize tasks, manage multiple responsibilities, and maintain a high level of productivity.
  • Strong problem-solving and critical thinking skills, with a focus on resident-centered care.
