Key Responsibilities- Provide comprehensive care to clients in their homes and communities, working collaboratively with other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments of clients' physical, emotional, and social needs, and develop care plans that address these needs.
- Lead the care planning process, working with other disciplines to complete and implement interdisciplinary client care plans.
- Facilitate regular communication with clients, families, and caregivers to ensure that their needs are being met and that they are receiving the best possible care.
- Collaborate with community resources and services to ensure that clients receive the support they need to maintain their independence and quality of life.
- Develop and maintain strong relationships with clients, families, and caregivers, providing education and support to help them navigate the healthcare system.
- Participate in ongoing education and professional development to stay current with best practices and evidence-based care.
- Registered in good standing with the College of Nurses of Ontario.
- Bachelor's Degree in nursing preferred.
- Canadian Nurses Association (CNA) community nursing certification is an asset.
- Minimum of two years of nursing experience, preferably in home and community care or a related field.
- Experience working with diverse client populations and in a community-based setting is an asset.
- Strong assessment and clinical skills, with the ability to work independently and as part of a team.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and caregivers.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
