Job Summary

We are seeking a skilled and compassionate Registered Practical Nurse to join our team at Southbridge Care Homes. As a key member of our healthcare team, you will be responsible for providing high-quality care to our residents, working collaboratively with our multidisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Assess the needs of individual residents and develop personalized care plans to promote maximum independence and well-being.
  • Provide direct resident care, including administering medications, performing medical procedures, and assisting with daily living activities.
  • Supervise and mentor unregulated care providers and attendants to ensure high-quality care is delivered.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
Requirements
  • Current Ontario Certificate of Competence from the College of Nurses of Ontario.
  • Medication administration certificate.
  • Ability to follow oral and written instructions in the English language.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a multidisciplinary team.
