Job Summary:

The Care Coordinator is a key member of the Home and Community Care Support Services team, responsible for managing a caseload of diverse patients with varying health concerns. The successful candidate will assess patient care needs, develop and implement care plans, and coordinate home and community care services.

Key Responsibilities:

  • Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs
  • Collaborate with patients, caregivers, and healthcare teams to develop individualized care plans
  • Coordinate home and community care services, including medical equipment and supplies
  • Facilitate communication among healthcare team members to maximize patient outcomes
  • Provide regular reports to physicians, service providers, and patients as required

Requirements:

  • Registered Nurse (BScN or diploma) or equivalent degree in a related field
  • Related professional experience, preferably in a community-based setting
  • Knowledge of medical diagnoses, symptom management, and case management principles
  • Fluency in English and French is required
