Job Summary

At Northern Health Authority, we are seeking a skilled Community Health Worker II to join our team. As a Community Health Worker II, you will provide home support services to clients in accordance with a pre-established care plan. Your key responsibilities will include assisting with activities of daily living, promoting maximum independence, and ensuring the well-being of individuals and families.

About the Role

This is an exciting opportunity to work in a well-integrated facility creating a cohesive care team. Our hospital and health centre provides a range of services, including emergency care, medical imaging, mental health and addictions support, home and community care, and medical clinic services.

Requirements

To be successful in this role, you will need to have a graduation from a recognized Home Support Attendant/Resident Care Aid Program, registration with the BC Care Aid & Community Health Worker Registry, and a valid Class 5 BC Drivers' License. You will also need to have certificates in CPR, First Aid, and Food Safe, as well as the ability to use a personal vehicle. If you have equivalent combination of current education, training, and experience, we encourage you to apply.

Skills and Abilities

As a Community Health Worker II, you will need to have the physical ability to perform the duties of the position, as well as the ability to establish and maintain rapport with clients, observe and recognize changes in clients, and communicate verbally and in writing with clients, family, and care team. You will also need to be able to work independently and in cooperation with others, organize and prioritize work, and operate related equipment. Finally, you will need to have home management skills and the ability to promote independent living while in the community.
