Job Overview:
Northern Health Authority seeks a skilled Primary Care Social Worker to join our team in Mackenzie, BC. As a key member of the interprofessional team, you will provide comprehensive assessment, care planning, and intervention for individuals and families in Primary Care Homes.

About the Role:
In this exciting opportunity, you will work collaboratively with other healthcare providers to deliver direct psychosocial care, counseling, group work, system navigation, advocacy, education, and linkage to relevant resources. Your primary responsibility will be to provide social work services to maintain or improve the health and functioning of individuals, families, and communities.

Requirements:
To be successful in this role, you must have a Bachelor's Degree in Social Work from an accredited university and two years recent related experience in a healthcare setting or a combination of education, training, and experience. You should also be registered with the British Columbia College of Social Workers and possess a current valid BC driver's license.

Key Responsibilities:
Assessment and treatment: Demonstrate ability to complete initial and ongoing assessments and utilize appropriate social work interventions. Teaching: Ability to teach and coach one-on-one and in groups. Knowledge integration: Integrate best practice and current research evidence to support professional practice decisions and actions. Communication: Effectively communicate with individuals, families, public, physicians, and interdisciplinary teams. Critical thinking: Problem solve demonstrating critical thinking and decision-making skills. Human caring and relationship-centered practice: Promote person-centered care while maintaining sensitivity to diverse cultures and preferences.
