Job Summary:
We are seeking a skilled and compassionate Primary Care Social Worker to join our team at Northern Health. As a member of our interprofessional team, you will provide comprehensive assessment, care planning, and intervention services to individuals and families in our Primary Care Homes.

Key Responsibilities:
• Provide psychosocial care, counseling, and group work to support clients in achieving their health and well-being goals.
• Collaborate with healthcare providers and the interprofessional team to coordinate care and services.
• Develop and implement person-centered care plans that address the unique needs of each client.
• Provide education and linkage to relevant community resources to support client self-management.
• Participate in quality improvement initiatives and contribute to the growth and development of the Primary Care Social Work team.

Requirements:
• Bachelor's degree in Social Work from an accredited university.
• Two years of recent related experience in healthcare or a combination of education, training, and experience.
• Current registration with the British Columbia College of Social Workers.
• Valid BC driver's license.
