Job Summary
Northern Health Authority seeks a skilled Community Health Worker II to join our team. As a key member of our healthcare team, you will provide home support services to our clients in accordance with a pre-established care plan. Your duties will include assisting with activities of daily living such as bathing, grooming, and oral hygiene, while promoting maximum independence. You will also perform delegated tasks for which delegation of tasks training has been completed. This role requires a strong commitment to ensuring the well-being of individuals and families, and promoting independent living while in the community.

About the Role
In this role, you will work as part of an interprofessional healthcare team under the direction of the Team Lead or designated clinical leader. You will provide home support services to clients in accordance with a pre-established care plan, and assist with activities of daily living such as bathing, grooming, and oral hygiene. You will also perform delegated tasks for which delegation of tasks training has been completed. A key component of this job is to ensure the well-being of individuals and families and to promote independent living while in the community.
