About the Role

We are seeking a highly skilled and compassionate Forensic Community Specialist to join our team at BC Mental Health & Substance Use Services in Victoria, BC.

Key Responsibilities

  • Conduct comprehensive biopsychosocial assessments and develop integrated treatment plans for clients with severe and persistent psychiatric illness.
  • Provide community case coordination and advocacy services, ensuring continuity of care and collaboration with mental health teams, private psychiatrists, and social service agencies.
  • Maintain accurate and concise documentation of client information, adhering to FPSC policy standards and regulatory requirements.
  • Provide nursing services, information, and support, including counseling and therapeutic support, medication management, and education on mental illness.
  • Facilitate access to psychiatric and social services, including referrals, accompaniment to community resources, and home visits as per the ITP and FPSC policy.

Requirements

  • Diploma in Nursing or Psychiatric Nursing.
  • Two years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
  • Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
  • Valid BC Drivers License.
  • Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action (2015), In Plain Sight Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and other related reports.
