About the Role

We are seeking a highly skilled and compassionate Forensic Community Specialist to join our team at PHSA. As a key member of our team, you will play a vital role in providing high-quality care to individuals with complex mental health and substance use challenges.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of our clients.
  • Provide case coordination and advocacy services to ensure seamless transitions between healthcare providers and community resources.
  • Maintain accurate and concise documentation of client information, ensuring compliance with regulatory requirements.
  • Collaborate with multidisciplinary teams to provide holistic care and support to clients and their families.
  • Develop and maintain strong relationships with community resources and healthcare providers to ensure access to necessary services and support.

Requirements

  • Diploma in Nursing or Psychiatric Nursing.
  • Two (2) years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
  • Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
  • Valid BC Drivers License.
  • Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action (2015), In Plain Sight Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and other related reports.
