Job Summary

We are seeking a highly skilled and compassionate Mental Health Nurse to join our team at PHSA. As a Forensic Community Specialist, you will play a critical role in providing high-quality care to individuals with complex mental health and substance use challenges.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of each client.
  • Provide case coordination and advocacy services to ensure seamless transitions between healthcare providers and community resources.
  • Maintain accurate and concise documentation of client information, including progress notes and treatment plans.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support for clients.
  • Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services and support.

Requirements

  • Diploma in Nursing or Psychiatric Nursing.
  • Two (2) years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
  • Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
  • Valid BC Drivers License. Local area travel may require the use of a personal vehicle.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC and BC's Declaration on the Rights of Indigenous Peoples Act.
