Patient Access Navigator, Central Intake Specialist

We are seeking a highly skilled and compassionate Patient Access Navigator to join our team at Nova Scotia Health Authority. As a key member of our Mental Health and Addictions team, you will play a critical role in facilitating access to mental health and addiction services for individuals across the province.
The Patient Access Navigator will be responsible for:
- Utilizing screening tools and risk assessments to identify client needs and develop effective care plans
- Providing brief supportive counseling and psychoeducation to clients and their families
- Collaborating with community resources and mental health teams to ensure seamless care transitions
- Building strong relationships with clients and their families to support their mental health and addiction journey
- Staying up-to-date on community resources and programs to provide clients with the best possible support
We are looking for a candidate with a strong background in social work and experience working with individuals with mental health and addiction concerns. About YouWe are seeking a candidate with:
- A Bachelor's degree in Social Work
- Registration with the Nova Scotia College of Social Workers
- Three years of experience working in mental health and addiction services
- Strong motivational interviewing skills and engagement techniques
- Experience utilizing screening and triaging tools
- Knowledge of harm reduction strategies and stages of change theory
- Demonstrated awareness of community resources and programs
- Demonstrated knowledge of recovery-oriented practice and patient-centered care

Hours and CompensationThis is a permanent full-time position with a salary of $42.89 per hour. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
