About the Opportunity

We are seeking a highly skilled and compassionate Patient Access Navigator to join our team at Nova Scotia Health Authority. As a key member of our Mental Health and Addictions team, you will play a critical role in facilitating access to mental health and addiction services for individuals across the province.

The Patient Access Navigator will be responsible for:

  • Utilizing screening tools and risk assessments to identify client needs and develop effective care plans
  • Providing brief supportive counseling and psychoeducation to clients and their families
  • Collaborating with community resources and mental health teams to ensure seamless care transitions
  • Building strong relationships with clients and their families to support their mental health and addiction journey
  • Staying up-to-date on community resources and programs to provide clients with the best possible support

We are looking for a candidate with a strong background in social work and experience working with individuals with mental health and addiction concerns. About You

We are seeking a candidate with:

  • A Bachelor's degree in Social Work
  • Registration with the Nova Scotia College of Social Workers
  • Three years of experience working in mental health and addiction services
  • Strong motivational interviewing skills and engagement techniques
  • Experience utilizing screening and triaging tools
  • Knowledge of harm reduction strategies and stages of change theory
  • Demonstrated awareness of community resources and programs
  • Demonstrated knowledge of recovery-oriented practice and patient-centered care

Hours and Compensation

This is a permanent full-time position with a salary of $42.89 per hour. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
