About the Role

We are seeking an experienced and skilled professional to join our team as a Renal Transition Coordinator Specialist. This is a unique opportunity to make a meaningful impact on the lives of renal patients and their families.

Key Responsibilities
  • Facilitate patient transfers between healthcare facilities and renal units, ensuring a smooth transition process for patients.
  • Liaise with interdisciplinary teams, including healthcare providers, patients, and families, to coordinate care and ensure optimal outcomes.
  • Develop and maintain effective working relationships with stakeholders, including Indigenous partners and community organizations.
  • Participate in the assessment and decision-making processes to optimize resource utilization and ensure effective patient care.
  • Provide education and support to patients and their families on dialysis therapy, treatment options, and related health information.
Requirements

To be successful in this role, you will need:

  • Four years of related experience in renal health, peritoneal dialysis, and hemodialysis.
  • Knowledge and experience with electronic health records used in the patient transfer process.
  • A valid driver's license and access to a vehicle for travel to patients' homes and healthcare facilities.
  • Bachelor of Nursing degree or equivalent qualification.
  • CRNM registration and Manitoba Nephrology Nursing Course completion.

  • An estimated salary of $65,000 - $85,000 per year, depending on experience.
