Job Summary

We are seeking a highly skilled and compassionate Social Services Coordinator to join our team at Kensington Health Centre. As a key member of our behavioral support team, you will play a vital role in supporting our residents who require complex behavioral support.

Key Responsibilities
  • Collaborate with the Behavioral Support Lead to provide comprehensive support to residents with complex behavioral needs.
  • Participate in the Behavioral Support Team and work closely with external partners to ensure seamless behavioral management.
  • Respond to Inquiries regarding Long Term Care and the Kensington Health Centre, providing accurate information and support.
  • Identify Psychosocial Challenges and develop strategies to address them in conjunction with the Director of Client Services.
  • Develop and Maintain Relationships with Discharge Planners, Social Workers, and community agencies to ensure effective support for residents.
  • Conduct Community Visits to assess the needs of prospective residents and provide support as needed.
  • Assess and Evaluate Residents using psychosocial assessments, Mini Mental State Exams, and other health assessment tools.
  • Provide Counseling and Support to residents and their families to address behavioral concerns and adjust to living in the Home.
  • Participate in Interdisciplinary Team Conferences and consult with the multi-disciplinary team to develop comprehensive treatment plans.
  • Coordinate Treatment Programs and make referrals as necessary to ensure residents receive the support they need.
  • Assist with 21-Day Assessments and ensure financial arrangements are made for new residents.
  • Coordinate Family Council and facilitate support groups to meet the needs of residents and their families.
  • Ensure Financial Arrangements are made for residents, including review and signing of Admission Contracts and Uninsured Services Forms.
  • Act as a Resource for Resident Advocacy and provide support with financial matters and application forms.
  • Assist with Power of Attorney Kits and ensure financial responsibility is maintained.
  • Participate in Evidence-Based Research to identify strategies for improving psychosocial interventions and resident outcomes.
  • Monitor the Environment for internal and external factors impacting care and ensure compliance with regulations and legislation.
Requirements
  • Minimum 3 Years' Experience in Long Term Care with specialized knowledge of Gerontology, Alzheimer Dementia, Behavioral Disorders, Complex and Palliative Care.
  • Minimum 2 Years' Experience working with older adults who experience responsive behaviors and those who are severely cognitively impaired.
  • Master's Degree in Social Work (MSW) and in good standing certificate with OCSWSSW.
