Job Summary

We are seeking a highly skilled and compassionate Social Services Coordinator to join our team at Kensington Health Centre. As a key member of our behavioral support team, you will play a vital role in supporting our residents who require complex behavioral support.

Key Responsibilities
  • Collaborate with the Behavioral Support Lead to develop and implement effective support plans for residents with complex behavioral needs.
  • Work closely with external partners, including community agencies and healthcare providers, to ensure seamless care coordination.
  • Provide counseling and support to residents and their families, addressing behavioral concerns and adjusting to living in our care home.
  • Conduct thorough assessments and evaluations to identify psychosocial needs and develop comprehensive treatment plans.
  • Participate in interdisciplinary team conferences and consult with the multi-disciplinary team to ensure effective psychosocial management of residents.
  • Develop and maintain relationships with community agencies, including CCAC, GMOT, and BSO, to ensure access to necessary resources and services.
  • Conduct community visits to assess the needs of prospective residents and provide support to families.
  • Provide expertise in evaluating resident needs for counseling services and make referrals as necessary.
  • Formulate and coordinate comprehensive treatment programs to meet the needs and expectations of residents and their families.
  • Assist in the completion of 21-day assessments for all new residents.
  • Coordinate Family Council and act as a liaison between Family Council and the Home, facilitating support groups as needed.
  • Ensure financial arrangements are made for each newly admitted resident, including review and signing of Admission Contracts and Uninsured Services Forms.
  • Act as a resource for resident advocacy for financial matters and completion of application forms, including ODSP, CPP, OAS, and GIS.
  • Assist residents and families in administering Power of Attorney (POA) kits and ensuring financial responsibility.
  • Participate in and utilize evidence-based research to identify strategies for improving psychosocial interventions, best practices, and resident outcomes.
  • Provide ongoing evaluation of the quality and effectiveness of psychosocial interventions.
  • Monitor the environment for internal and external factors impacting care, including government regulations, key associations, and competitive intelligence.
  • Adhere to relevant legislation, regulations, and standards, including the Long Term Care Homes Act, 2007, and the Resident's Bill of Rights.
Requirements
  • Minimum three (3) years' experience in Long Term Care with specialized knowledge of Gerontology, Alzheimer Dementia, Behavioral Disorders, Complex and Palliative Care.
  • Minimum 2 years' experience working with older adults who experience responsive behaviors.
  • Minimum 2 years' experience working with older adults who are severely cognitively impaired.
  • Minimum Masters of Social Work (MSW) degree and in good standing certificate with OCSWSSW.
