About the Role

We are seeking a highly skilled and compassionate Social Services Coordinator to join our team at the University Health Network. As a key member of our Allied Health team, you will play a vital role in providing exceptional patient care and support.

Key Responsibilities
  • Assessment and Intervention: Conduct thorough assessments of patients to identify their needs and develop personalized intervention plans to promote positive outcomes.
  • Collaboration and Communication: Work closely with interprofessional teams, patients, and families to facilitate effective communication and coordination of care.
  • Education and Training: Provide clinical teaching and supervision to students and colleagues to enhance their knowledge and skills.
  • Research and Quality Improvement: Participate in research activities and quality improvement initiatives to advance the field of social work and improve patient care.
Requirements
  • Education: Master's degree in Social Work (M.S.W) or equivalent.
  • Registration: Registration with the Ontario College of Social Workers and Social Service Workers.
  • Experience: 2-3 years of experience working with an interprofessional team in a healthcare setting.
  • Skills: Strong knowledge of policies, legislation, and programs related to social welfare institutions and services.
