Carepath Intake Coordinator

About the Role:
The Carepath Intake Specialist will provide exceptional client service by delivering a comprehensive understanding of the Carepath and Bayshore programs. This role requires a compassionate and empathetic individual who can portray a strong knowledge of positioning and selling solutions to clients and their families.
Key Responsibilities:
- Respond to client inquiries regarding the Carepath program, providing information and answering questions.
- Verify client eligibility and enroll them in the program, ensuring all required data fields are completed.
- Notify the Carepath Administrator and Director of the program, as well as send welcome packages and email consents.
- Manage the inquiry process, providing information about Bayshore services to families and booking assessments with local Home Health branches.
- Support internal and external audit processes, manage information flow, and adhere to Bayshore and Carepath policies and procedures.
Requirements:
- University/College education or equivalent combination of education and experience.
- Experience within a call center/customer service environment, with exceptional interpersonal and communication skills.
- Ability to work independently and as part of a team, with strong computer and typing skills.
- Knowledge of personal computers and related software, including Windows, Word, and Excel.
