Job Summary

Mt. Carmel Clinic is seeking a skilled Outreach Worker to join our Community Wellness team. As a key member of our coordinated service delivery team, you will work proactively with new and existing clients, offering a range of support and recovery services. Your role will involve building relationships, advocating for clients, and providing services in a non-judgmental and culturally appropriate manner.

Key Responsibilities

  • Provide outreach services to hard-to-reach populations
  • Develop and implement service plans to support clients
  • Build and maintain relationships with clients, community partners, and other stakeholders
  • Provide education and support to clients on health and wellness topics
  • Collaborate with interdisciplinary teams to ensure comprehensive client care

Requirements

  • Minimum 2 years' experience working in community settings
  • Experience working with hard-to-reach populations
  • Valid driver's license and access to a reliable vehicle
  • Post-secondary education in a Social Services field (an asset)
