Job Summary

The Mount Carmel Clinic is seeking a skilled Community Outreach Specialist to join our team. As a key member of our coordinated service delivery team, you will work proactively with new and existing clients, offering a range of support and recovery services. Your primary focus will be on building relationships and advocating for clients, providing services in a non-judgmental and culturally appropriate manner.

Key Responsibilities

  • Develop and maintain relationships with clients, understanding their needs and goals
  • Provide support and recovery services, working within existing service plans and on an as-needed basis
  • Advocate for clients, promoting their well-being and empowerment
  • Collaborate with the interdisciplinary team to ensure comprehensive care
  • Participate in ongoing training and professional development to enhance skills and knowledge

Requirements

  • Minimum of 2 years' experience working in community, preferably with vulnerable populations
  • Proven experience in outreach and advocacy, with a focus on relationship-building and critical thinking
  • Knowledge and understanding of the Housing First model and Assertive Community Treatment (ACT) methodology
  • Excellent communication and interpersonal skills, with the ability to work in a fast-paced environment
  • Valid driver's license and access to a reliable vehicle

Preferred Qualifications

  • Post-secondary education in a Social Services field
  • Experience working with Indigenous populations and understanding of Indigenous culture and traditions
  • Knowledge of social determinants of health, population health, harm reduction, trauma-informed, and community development principles
