Job Summary

We are seeking a highly skilled and compassionate Community Outreach Worker to join our team at Mount Carmel Clinic. As a key member of our coordinated service delivery team, you will work proactively with new and existing clients, offering a range of support and recovery services.

Key Responsibilities
  • Provide outreach services to hard-to-reach populations, building relationships and trust with clients.
  • Develop and implement individualized service plans, working collaboratively with clients to achieve their goals.
  • Provide advocacy and support services, promoting the health and well-being of clients.
  • Work within an interdisciplinary team environment, collaborating with colleagues to achieve service goals.
  • Apply knowledge and understanding of Indigenous culture and traditions, as well as the impact of colonization on the health and well-being of Indigenous individuals and families.
Requirements
  • Minimum of 2 years' experience working in community, preferably with the program's target population.
  • Experience providing outreach to hard-to-reach populations and working with vulnerable populations.
  • Knowledge and understanding of the Housing First model and Assertive Community Treatment (ACT) methodology.
  • Proven ability to work within an interdisciplinary team environment, with excellent communication and interpersonal skills.
  • Commitment to the principles that guide Mount Carmel Clinic, including diversity, equity, cultural safety, and social determinants of health.
