Job Summary

The Community Outreach Specialist will work as part of the coordinated service delivery team at Mount Carmel Clinic. This role involves working proactively with new and existing clients, offering a range of support and recovery services. The successful candidate will be a relationship-builder and advocate, providing services in a non-judgmental and culturally appropriate manner.

Key Responsibilities

  • Provide outreach services to hard-to-reach populations
  • Work with a vulnerable population, providing support and advocacy
  • Develop and maintain relationships with clients, helping them to build new skills and achieve their goals
  • Collaborate with an interdisciplinary team to provide comprehensive services
  • Apply knowledge and understanding of the Housing First model and Assertive Community Treatment (ACT) methodology

Requirements

  • Minimum of 2 years' experience working in community, preferably with the program's target population
  • Valid Drivers' license and access to a reliable vehicle
  • Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills
  • Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes
  • Commitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission, and values
  • Knowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families
  • Strong knowledge, commitment, and demonstrated ability to apply diversity, equity, cultural safety, pro-choice, social determinants of health, population health, harm reduction, trauma-informed, and community development principles in all aspects of the work

Education and Experience

  • Post-secondary education in a Social Services field
  • Minimum of 2 years' experience working in community, preferably with the program's target population

Selection Criteria

  • Candidates will be selected based on their experience, skills, and ability to work with vulnerable populations
  • Applicants may undergo testing to determine the knowledge, skill, and ability required for the position
