Job Summary

We are seeking an experienced Registered Nurse (RN) to join our dynamic home and community care team. In this rewarding role, you will have the opportunity to make a meaningful difference in the lives of clients by providing comprehensive care in an interdisciplinary setting.

Responsibilities
  • Provide comprehensive care in an interdisciplinary team environment
  • Serve as primary contact for clients when designated as primary care provider/clinician
  • Support clients, families, and caregivers through care transitions by collaborating and connecting them with community support services
  • Lead discharge and transition care planning discussions during regular integrated team meetings
Requirements
  • Bachelor's Degree in nursing preferred
  • Canadian Nurses Association (CNA) community nursing certification is an asset
  • Minimum of two years of nursing experience
  • Experience in one of the following areas: home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
Benefits
  • Competitive salary range: $60,000 - $80,000 per year, based on experience and qualifications
  • Guaranteed 37.5 hours per week with 12-hour shifts

  • A vehicle and valid driver's license required for travel
