Salary Information

This is a full-time, permanent position offering a guaranteed 37.5 hours per week, with 12-hour shifts. The estimated annual salary for this role is approximately $83,000, based on industry standards and your extensive experience as a Registered Nurse. This figure may vary depending on your location, qualifications, and performance.

Job Description

You will provide comprehensive care to clients in their homes, working collaboratively with interdisciplinary teams to develop and implement holistic care plans. Your expertise and clinical leadership skills will enable you to lead care planning processes, facilitate regular communication among team members, and ensure seamless transitions for clients.

  • Provision of comprehensive care in an interdisciplinary team environment
  • Primary contact for clients when designated as the primary care provider/clinician
  • Continuous support to the team in delivering care for clients
  • Client assessment and development of interdisciplinary care plans based on best evidence/practices and individual needs
Required Skills and Qualifications

To succeed in this role, you will require:

  • Registration in good standing with the College of Nurses of Ontario
  • Bachelor's Degree in nursing (preferred)
  • Canadian Nurses Association (CNA) community nursing certification (asset)
  • Minimum of two years of nursing experience
  • Experience in one of the following areas: home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
