About the Role

In this position, you will:

  • Provide comprehensive care as an integral member of an interdisciplinary team
  • Serve as primary contact for clients when designated as the primary care provider/clinician for the client's team
  • Offer continuous support to your team in delivering exceptional care to clients
  • Conduct thorough client assessments and develop tailored care plans based on best practices and individual needs
  • Lead the care planning process, working collaboratively with other disciplines to implement comprehensive client care plans
  • Facilitate regular communication among team members, including huddles, rounds, and status updates, when designated as primary care provider/clinician
  • Participate in discharge and transition care planning discussions during integrated team meetings, as needed
  • Organize family and caregiver meetings to ensure seamless care transitions
  • Collaborate with community resources to support clients' health and well-being

We're seeking a registered nurse in good standing with the College of Nurses of Ontario, preferably holding a Bachelor's degree in nursing. Canadian Nurses Association (CNA) community nursing certification is an asset. A minimum of two years of nursing experience, particularly in home/community care, acute care, chronic disease management, health promotion, palliative care, or mental health, is required. Diverse nursing experiences with different client groups are valued. Demonstrated clinical leadership skills, enthusiasm for community rehabilitative care, and excellent assessment skills are essential.

Compensation and Benefits
