Dynamic Community Care Clinician

About the Role:
We are seeking a skilled Registered Nurse to join our dynamic team in providing high-quality care to clients in their homes and communities. As a member of our interdisciplinary care team, you will be responsible for delivering comprehensive care, leading care planning processes, and collaborating with other disciplines to ensure seamless transitions and optimal outcomes for our clients.
Key Responsibilities:)
- Provide continuous and comprehensive support to your team in the delivery of care for your clients
- Complete client assessments and develop interdisciplinary care plans based on best evidence/practices and client needs
- Lead the care planning process and work with other disciplines to implement client care plans
- Facilitate regular interdisciplinary team communication and lead discharge and transition care planning discussions
- Support clients, families, and caregivers in developing strong self-care strategies and optimizing health through effective health promotion
Requirements:
- Registered in good standing with the College of Nurses of Ontario
- Bachelor's Degree in nursing preferred
- Canadian Nurses Association (CNA) community nursing certification is an asset
- Minimum of two years of nursing experience
- Experience in home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
