We are seeking a dedicated and compassionate Registered Nurse to join our team at SE Health, a not-for-profit social enterprise that delivers high-quality care to clients across Canada. About the Role:

This full-time permanent position offers 37.5 hours of work per week, with guaranteed shifts of 12 hours each. As a Registered Nurse, you will be responsible for delivering comprehensive care to clients in their homes and communities. Your primary focus will be on providing personalized support to clients, developing and implementing individualized care plans, and collaborating with interdisciplinary teams to achieve optimal health outcomes.

Key Responsibilities:
  • Provide continuous and comprehensive support to clients in their homes and communities
  • Develop and implement individualized care plans based on best evidence and client needs
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal health outcomes
  • Facilitate regular communication with clients, families, and caregivers to ensure informed decision-making and empowerment
  • Support clients in developing strong self-care strategies and promoting healthy lifestyle choices
Requirements:
  • Registered in good standing with the College of Nurses of Ontario
  • Bachelor's Degree in nursing preferred
  • Minimum of two years of nursing experience
  • Experience in home/community care, acute care, chronic disease management, or related fields
  • Excellent assessment skills, clinical judgment, and ability to work independently and collaboratively as part of a team
  • Vulnerable sector check (current)
What We Offer:

At SE Health, we offer a competitive salary range of $85,000 - $105,000 per year, depending on experience, plus benefits, pension, and opportunities for professional growth and development.
